Mowlam is the only service presently available to provide a specific rehabilitation service in the required timeframe, this is due to a combination of factors such as: 1. Specialised Expertise: Mowlam Healthcare has a proven track record of offering specialised rehabilitation services tailored to the specific needs of the patient population. For instance, they specialise Level 2 & 3 Rehabilitation inclusive of post specific illness or injury eg. stroke, hip fracture, trauma. Reversible & modifiable functional deficit/disability associated with eg. Chronic Disease, Cancer, Dementia, Frailty, HAD etc., which other providers may not offer. In response to demographic changes, the HSE is implementing a raft of reforms centred on developing an Age Friendly Health System AFHS. This evidence-based approach is based on the 4Ms Medication, Mobility, Mental Health and What Matters to You. This provides and overarching framework for the care of older people across the care journey irrespective of diagnosis or care location. The 4Ms is central to multiple simultaneous reforms which are taking place at pace and scale in the health system. This substantively includes. Urgent and Emergency Care in ED (eg >75yr 24hr Breaches), Inpatient Care Pathways (NCP OP), Post-Acute Rehabilitation (Draft -Rehab Mapping Report 2024), Integrated Nursing Home Care (Pall Care, PoLL, Dementia, NCP OP, CHN), Introduction of an Age Friendly Health System (4Ms) (ECC, NAS, NCP OP) and Embedding new models through the integration of CHN, CST OP, Health and Wellbeing, NAS, HAFH. In parallel, Mowlam is working with the Institute for Health Improvement (IHI) and is a participant in the Age Friendly Health System Community of Practice. 30% of Mowlam sites have commenced working towards AFHS accreditation in 2024 with the remainder to join in 2024/5. Molwam will become the first age friendly health system providing residential care and rehabilitation outside of the USA by 2025. 2. Geographic Availability: To the best of my knowledge currently Mowlam are the only provider offering these services in our geographic area, making them the only practical option for patients transfers from CUH, MUK, UHK, MGH, BGH, SIVUH who cannot travel long distances due to medical or financial constraints. 3. Proven Outcomes: Mowlam have demonstrated superior outcomes, backed by data or testimonials, indicating that their services are uniquely effective in helping patients recover and regain independence. 4. Accreditation and Compliance: Mowlam Healthcare has over 150 years combined industry experience, operate 30 Long Term Residential Care Facilities, employ over 1,700 clinical staff and provide over 1,700 specialist care beds for older people. Mowlam is the largest Irish private residential care operator in Ireland and operates a Continuum of Care Model. This is designed to support people who need additional care, or specialised services, by offering seamless transitions through levels of clinical care, as a person’s health care needs change. They also have met and continue to meet all the regulatory standards (NICE, HIQA, NSSBHC, H&S etc) that are essential for delivering the service, distinguishing them in the market. 5. Integrated Services: Mowlam offer a comprehensive suite of services, such as physical therapy, occupational therapy, and psychosocial, ensuring holistic care. 6. Capacity and Resources: Other providers lack the necessary resources, such as specialised trained staff, making Mowlam the only viable option to meet the demand for this service.
